Occupational Therapist required to adhere to the College of Occupational Therapy Code of Ethics & Professional Conduct and the Health & Care Professions Council Standards of Proficiency (Code & Standards) within their role. An understanding of the practice areas appropriate to the level of Occupational Therapist is an integral part of this role.
Purpose of job
- To provide advice and information relating to the provision of Social Care for Adults / Older People / Children. Undertake social care assessment, care planning, monitoring and review service for service users with highly complex needs within the Prison Cluster, using an integrated case management approach.
- To provide advice and information under the Care Management/Occupational Therapy Process for Adults / Older People / children which could lead to service provision or referral on to other departments, agencies and/or organisations
- Interventions aim to enhance the person’s independence, safety and quality of life via rehabilitation, Moving & Handling, local / out of borough hospitals / voluntary sector specification of equipment and/or structural changes to the environment.
- To liaise and promote the continuity of service provision.
- To undertake appropriate safeguarding work within Adults / Older People / children.
- To ensure that all work complies with Council policies and procedures and that statutory obligations / professional standards / code of conduct requirements are met.
- To take responsibility for maintaining own registration with the professional regulator (HCPC)
